Form 1095-C Has Been Mailed
Form 1095-C provides information about the health coverage offered by your employer and, in some cases, about whether you enrolled in this coverage. Anyone who was employed by Northwestern University in calendar year 2023 who was eligible for employee health insurance will receive a Form 1095-C. Addtionally, retirees enrolled in the Legacy Retiree PPO will receive a 1095-C.
Form 1095-C are mailed the week of Monday, January 15, 2024. It can take 2- to 3-weeks from this date to receive your 1095-C.
